I would love to see those case, privately if not on this forum. DCB failures are rare. But they do happen. Each case has something to teach us. I am keeping a mental list of things that became apparent to the operator(s) after such an event. The list: 1. Unrecognized guide dissection 2. Delayed loading with DAPT (in the holding area) 3. Extensive Ca and recoil (+/- unrecognized IMH) 4. Unrecognized IMH 5. Unrecognized acute stent thrombosis (MB) after SB DCB during OMI 6. Long spiral dissection (always post NCB, never CBA) covering several SBs. I am sure there are others. But nothing gets added to this list without angio or IVUS evidence! 😂 Also, these are not my cases... for now!

The difference in coronary engagement was striking. That night made me appreciate a practical advantage of SAPIEN easier coronary access.
