I think what he is saying is there are many Muslims who have accepted Jesus Christ as their lord but are worried about what people would say if they boldly declare they are no longer Muslim faithful. I once know an Imam (not sure if he was an imam but a very strong Muslim) who often come to see my uncle who is a pastor for prayers. They became friends afterwards and share lots of thing together. This imam built a big mosque beside his house in his street. He told my uncle that he accepts Jesus Christ but what would people in his street who has known him to be strong Muslim say if he boldly declare. He was also contemplating if he would have to go close down his mosque or something. This was a big issue for him so he kept worshipping in his heart and kept the physical act of being a Muslim. He also comes for vigil most times in church.
