Hi all -- had a request: looking for patients who've read their medical records and seen biased or racist language in their notes. Is that you? What happened? Did you speak with your doctor? #opennotes

After 18 years away (thanks, #braincancer), I'm going back to school! My major has now changed from Communications to Cognitive Science. Wish me luck! TL;DR: I'll still be working 2 days/week with #OpenNotes, and going to school full time. Details: #btsm

No podemos seguir hablando de empoderar o de ‘en el centro’ mientras no se pueda acceder a tu ficha (y no, no a través de la ley en 20 días en fotocopias ilegibles) #OpenNotes Sobre registro clínico, esto 👇🏼

Here it is: My official guide to the difference between clinical notes and the After Visit Summary. - When clinical notes are shared w/patients it is called #opennotes. Lots of evidence that notes are helpful for patients and care partners. - The After Visit Summary is a joke.

🤯 Today I learned that in the @apple Health app you can search all of your connected information… including your own progress notes! I was able to EASILY find my favorite note by a doctor with a simple text search. Why isn’t this in commercials by now? #opennotes

Dear Clinicians - Do you write "patient refused to" or "patient elected"? Do you write "patient complained of" or "patient reported"? Is @myopennotes changing the words you use? #OpenNotes Thx to @ChrisMoriates

Patients don’t “refuse” things you recommend, they “decline” or “prefer not to.” No need to make this a power struggle. Seeing yourself as a team w patients is more effective, and more satisfying for you #TipsForNewDocs #WordsMatter

I never get over how much I love reading old progress notes. One year ago: my palliative care doc noted how our cat, Mark, is a support to my husband. In an open note, I feel seen. #btsm #hapc #opennotes

There’s no need to panic. But there is a need to revise alert notification systems and nomenclatures, especially in the era of #OpenNotes
