𝐀𝐰𝐚𝐤𝐞 𝐈𝐬𝐧'𝐭 𝐄𝐧𝐨𝐮𝐠𝐡 @PanCAN #PANcan A #PancreaticCancer vaccine stirred the immune system. The case for $NWBO #DCVax-Direct and $MRK #Keytruda begins with what happens next. When Ralph Steinman was diagnosed with pancreatic cancer in 2007, the cell he had discovered became part of his own treatment. In 1973, Steinman and Zanvil Cohn had described an unusual cell with branching extensions. Steinman named it the dendritic cell. Years of experiments established its importance: it collected material, displayed pieces of it to other immune cells and helped direct their response. It stood at the hinge between noticing something and organizing an attack.¹ During the four and a half years after his diagnosis, Steinman received surgery, chemotherapy and several experimental immunotherapies, including treatments involving his own dendritic cells. His removed tumor supplied material for individualized approaches. He continued working almost until his death. Three days later, the Nobel committee announced the prize honoring his discovery.² His story gives today's pancreatic-cancer vaccine headline a deeper meaning. Stirring the immune system is one achievement. Helping it control cancer is another. ELI-002 7P is a ready-made peptide vaccine. It contains short protein fragments representing seven mutated forms of KRAS, a protein involved in controlling cell growth. An immune-stimulating ingredient helps generate responses in lymph nodes, where immune cells meet. After standard treatment, AMPLIFY-7P assigned 96 patients to vaccination and 48 to observation. None had recurrence visible on scans. Among 90 vaccinated patients evaluated, 89 developed stronger T-cell responses. Yet the trial's main comparison did not establish better protection against cancer recurrence or death.³ 𝐓𝐡𝐨𝐬𝐞 𝐟𝐢𝐧𝐝𝐢𝐧𝐠𝐬 𝐛𝐞𝐥𝐨𝐧𝐠 𝐭𝐨𝐠𝐞𝐭𝐡𝐞𝐫. Recognition increased. The trial’s main clinical goal was not met. T cells have different jobs. Some coordinate the response; others kill cells displaying a recognized target. A blood test can detect their response to cancer-related material. It cannot, by itself, show that they reached the remaining cancer, withstood its defenses and destroyed it. The vaccine also prompted responses to additional tumor targets in 13 of 15 selected strong responders. The question therefore reaches beyond simply counting targets: how does recognition become effective action?³ To understand that journey, follow a fragment of a cancer cell. A dendritic cell takes up material and breaks it into pieces. It displays some of those pieces on its surface for T cells to inspect. A recognizable piece is called an antigen. But recognition does not automatically authorize an attack. The presenting cell supplies additional signals that shape the response. Its own condition and the messages around it help determine whether the encounter promotes immunity or teaches the body to leave the material alone.¹ A helper T cell can recognize the display and send a signal back. This contact, called licensing, strengthens the dendritic cell's ability to support cancer-killing T cells. The helper helps prepare the instructor for its next encounter.⁴ Another message is IL-12, a protein supporting the coordinated cellular attack called type 1 immunity. Activated T cells can return a signal called interferon gamma, encouraging dendritic cells to produce IL-12. In experiments with tumors in mice, that two-way exchange supported the response to a drug that releases a brake on T cells. Each cell helped sustain the other.⁵ Immune instruction is a physical process: contact, a message, a response, and another message in return. A human-cell study published in 2012 showed why the presenter's preparation matters. Researchers kept the cancer-associated fragment the same while changing how dendritic cells were conditioned. Cells prepared with help from activated helper T cells generated stronger measured killing responses and T cells with features associated with immune memory, the capacity to respond again. 𝐊𝐞𝐞𝐩𝐢𝐧𝐠 𝐭𝐡𝐞 𝐭𝐚𝐫𝐠𝐞𝐭 𝐭𝐡𝐞 𝐬𝐚𝐦𝐞 𝐝𝐢𝐝 𝐧𝐨𝐭 𝐤𝐞𝐞𝐩 𝐭𝐡𝐞 𝐫𝐞𝐬𝐩𝐨𝐧𝐬𝐞 𝐭𝐡𝐞 𝐬𝐚𝐦𝐞.⁶ The receiving system also has a history. In AMPLIFY, prior radiation was associated with lower immune-cell counts and smaller vaccine responses. The patient's condition is part of the biological encounter.³ 𝐃𝐂𝐕𝐚𝐱-𝐃𝐢𝐫𝐞𝐜𝐭 𝐦𝐚𝐤𝐞𝐬 𝐭𝐡𝐞 𝐩𝐫𝐞𝐬𝐞𝐧𝐭𝐢𝐧𝐠 𝐜𝐞𝐥𝐥 𝐩𝐚𝐫𝐭 𝐨𝐟 𝐭𝐡𝐞 𝐭𝐫𝐞𝐚𝐭𝐦𝐞𝐧𝐭. Cells collected from the patient's blood are developed and partially activated outside the body, then injected into an accessible tumor. They are intended to acquire material there and help initiate an immune response. Preparation outside the tumor allows investigators to shape and measure the cells before administration.⁷ The tumor supplies the material. The prepared dendritic cells bring the machinery for processing it, presenting it and helping direct the response. A separate manufactured KRAS-peptide vaccine is not part of this approach. The phase I findings reveal the decisive distinction: 𝐃𝐂𝐕𝐚𝐱-𝐃𝐢𝐫𝐞𝐜𝐭 𝐰𝐚𝐬 𝐚𝐝𝐦𝐢𝐧𝐢𝐬𝐭𝐞𝐫𝐞𝐝 𝐰𝐢𝐭𝐡𝐨𝐮𝐭 𝐊𝐞𝐲𝐭𝐫𝐮𝐝𝐚. Across several advanced cancers, biopsies showed increased immune cells inside tumors in about 54% of assessed patients. Investigators identified helper and killer T cells and signs of local immune activity.⁷ The point is where the evidence was found. The peptide-vaccine headline concerns a response measured in blood. Direct's biopsies documented immune activity inside existing tumors, without a checkpoint drug. The same study found increased PD-L1 in 19 of 25 evaluated tumors. PD-L1 can engage PD-1, a receptor on T cells, and restrain their activity. The tissue showed an immune response alongside an increase in a signal capable of suppressing it.⁷ This creates a specific proposal: 𝐃𝐂𝐕𝐚𝐱-𝐃𝐢𝐫𝐞𝐜𝐭 𝐩𝐥𝐮𝐬 𝐊𝐞𝐲𝐭𝐫𝐮𝐝𝐚. Keytruda, also called pembrolizumab, blocks PD-1 so it cannot receive those inhibitory signals. Its proposed contribution is to release a brake on a response that Direct is designed to initiate.⁸ The biological activity observed with Direct alone is the starting point. Adding Keytruda would target a mechanism capable of restraining that activity. The aim is to turn an immune response already documented inside tumors into more effective, sustained cancer control. Steinman spent his career making the instructor visible. The case for DCVax-Direct and Keytruda takes that discovery seriously: the cell presenting the cancer and the signals restraining the response both belong in the treatment strategy. 𝐖𝐚𝐤𝐢𝐧𝐠 𝐭𝐡𝐞 𝐢𝐦𝐦𝐮𝐧𝐞 𝐬𝐲𝐬𝐭𝐞𝐦 𝐢𝐬 𝐚 𝐛𝐞𝐠𝐢𝐧𝐧𝐢𝐧𝐠. 𝐓𝐡𝐞 𝐣𝐨𝐛 𝐢𝐬 𝐭𝐨 𝐡𝐞𝐥𝐩 𝐢𝐭 𝐟𝐢𝐧𝐢𝐬𝐡. 𝐒𝐨𝐮𝐫𝐜𝐞𝐬 1. Nobel Prize scientific background: the discovery and function of dendritic cells. 2. The published preparation used GM-CSF, killed BCG and interferon gamma, with no added PGE2 listed. 8. National Cancer Institute: pembrolizumab and its PD-1-blocking mechanism.

A Pancreatic Cancer Vaccine Activated the Immune System in 89 Out of 90 Patients What if a vaccine could teach the immune system to recognize one of the deadliest cancers? Researchers tested an experimental vaccine called ELI-002 7P in 144 patients with pancreatic cancer who had already undergone surgery and treatment. The vaccine targets seven common KRAS mutations that help cancer cells survive. The results were remarkable: 89 out of 90 evaluable vaccinated patients developed stronger T-cell responses against these cancer-related mutations. These immune cells are responsible for recognizing and attacking abnormal cells. But there is an important twist. Despite the powerful immune response, the Phase 2 trial did not meet its main goal of significantly delaying cancer recurrence across all participants. Researchers are now investigating which patients may benefit most. It is an encouraging step toward personalized cancer vaccines, but much more research is needed before this approach can be considered an effective treatment. Source:
